Terms and Conditions:
 
     The person / agency contracting our agency agrees to make payment based upon the price quote given to them by Safety & Health Solutions, LLC.  The person / agency contracting SHS will be responsible for a fee of $150.00 for each service date or session canceled, if canceling within 72 hours of the service date(s).  The services listed above will not be officially scheduled until a contractual agreement is signed and returned to Safety & Health Solutions, LLC.  Students must attend the entire course unless other arrangements have been made ahead of time. Student participation does not guarantee the issuance of certification.  Students must successfully complete the program with a passing grade.  In the situation that a student does not pass, they will have to attend another program and pay the applicable attendance fee. Our instructors will recommend to you what level of training is required for your organization or level of employment.  If you elect an alternative level of training, other than that recommended by Safety and Health Solutions, LLC, and it is found to not be accepted by state or local guidelines, Safety and Health Solutions, LLC will not be liable for the cost of additional training needed for your compliancy.  The number of instructors provided by our agency is dependent upon the number of students scheduled to attend the course(s) outlined above. If the number of students decreases significantly, causing the unnecessary assignment of an instructor, the above listed agency will be charged a fee of $20.00 per hour for that instructor(s).  If the class size falls below our minimum requirement of four (4) students, the contracting agency will be charged for the required minimum.  Payment is requested within the specified payment terms on this agreement.  If payment is not received within the outlined payment date, and other payment arrangements have not been made, late fees will apply.  The following fee schedule will be used:

Outstanding Balances:
 
$1 to $250 = $25.00 late fee and a 5% monthly interest charge
$251 to $500 = $50.00 late fee and a 5% monthly interest charge
$501 or greater = $75.00 late fee and a 5% monthly interest charge

     Should the outstanding balance(s) need to be obtained through collections, the above listed, contracting party will be responsible for any and all collections / legal fees.  All legal disputes will be handled in the New Jersey court system.  A charge of $35.00 will be applied for returned checks.  Either party may reschedule a class or service date, without penalty, as a result of emergencies and/or inclement weather.  All products must be returned with 30 days and still be wrapped in the manufacturer’s sealed packaging unless the product has been found to be defective.  The product can either be replaced or reimbursement issued.  The customer is responsible for any shipping charges for products that are returned for reasons other than defect.
